As a person moves towards the end of their life, they may need extra support and care to manage medical conditions so that they can focus on spending more time with their family and on the things they enjoy. We offer end-of-life care in Christchurch & surrounding areas, in line with the Six Steps End-of-Life Accreditation.
In fact, we worked tirelessly to adapt the steps for community-based care and are setting a new standard for end-of-life care across the country.
Our carers are fully trained by our lead nurse to provide clinical care so your loved one doesn’t have to put up with a revolving door of care practitioners. We’re here to offer comprehensive and consistent support to you and your loved one during this challenging time.
What is end-of-life care?
People often don’t understand the difference between end-of-life care and palliative care. Palliative care is for people with life-limiting conditions and is also known as ‘supportive care’. The aim of palliative care is to provide treatment and support to ensure good quality of life. We can provide palliative care at any stage of life.
End-of-life care is specifically tailored for people who have been identified as reaching the end of their life and aims to help them manage physical symptoms and pain, as well as providing emotional, spiritual and psychological support for the individual, their family and friends.
Six-step Accreditation
The Six Steps to Success End-of-Life accreditation is a national scheme designed to support the delivery of high-quality end-of-life care. The steps are:
- Discussions as end-of-life approaches
- Assessment, care planning and review
- Coordination of care
- Delivery of high quality care in domiciliary care organisations
- Care in the last days of life
- Care after death
We follow these six steps in partnership with allied health professions to ensure that end-of-life care meets the needs and requirements of the individual.
